What is Root Cause Analysis

  • Root-Cause Analysis is “an analytical technique used to determine the basic underlying reason that causes a variance or defect or a risk”
  • Steps in conducting a session:–Explain the difference between causes and their effects using  examples. Point out that effects can’t be solved but underlying causes  can.

    –Divide a flip chart into two sections: causes and effects

    –Define the effect to be analyzed

    –Ask the group to identify all the causes

    –Employ a vision technique to develop possible ways to address the  causes (possible follow-up session).

    –To set priority on ideas, employ a decision technique to sort out the  best ideas (possible follow-up session)
