Consulted (C): This is the person or team who needs to be consulted during the task to ensure that it is completed correctly and to their satisfaction. They may provide input, feedback, or recommendations, but they do not have final authority over the task. For example, they might be stakeholders who need to approve the outcome of a task.

Informed (I): This is the person or team who needs to be kept informed of the task’s progress and outcome. They are not directly involved in the task, but they need to know what is happening for informational purposes. For example, they might be team members who need to know the status of a task so they can plan their own work accordingly.

It’s important to note that in a RACI matrix, each task should only have one person or team who is accountable. This helps to ensure that there is clear accountability and reduces the risk of confusion or misunderstandings. The RACI matrix can also be used to identify areas where additional resources or support may be needed, and to track progress towards completing the project.

To create a RACI matrix, a list of tasks and decisions for the project is created, and then for each task, the person or team who is Responsible, Accountable, Consulted, and Informed is identified.
