ROOT CAUSE ANALYSIS ( RCA )

Root Cause Analysis

It is a systematic or structured way of identifying “root cause of the problems or events” and provide a efficient solution. It included various techniques , methodologies etc.…., It works on common problems so that main problems can be identified. RCA  Shows there process , when system or process failed to identify the problems.

It have following goals :

First Goal

It finds the cause of the Problem and provide the solution.

Second Goal

It describes the methods to prevent the problems , like to compensate for the problems.

Third Goal

It ensures that this problem can’t affect us in future if the same problem arise.

It follows some principles:

• Realize that , there can be multiple causes , not only single cause of the problem.
• Don’t underestimate short term relief of the problem , it is also important.
• Focus on correcting and providing solution for the problem , not on symptoms of the problem.
• Focus on How and Why cause arises , instead of WHO is responsible .

It follows 5 WHY’s.
